Empowering Students for Independent, Purposeful Lives

Transition planning at Riverview doesn’t just prepare students for graduation; it prepares them for life. We’re here for the journey, supporting both students and their families every step of the way.

Through collaborative efforts involving families, students, staff, and outside providers,  students receive the tools and support they need to prepare for a happy and fulfilling life beyond school.

Comprehensive Student Support

Our comprehensive approach integrates academics, social-emotional growth, independent living skills, vocational training, and wellness to nurture the whole student. Students develop transferable real-world skills and confidence needed for the next steps, whether it’s further education, employment, or greater independence in daily life. 

Resources for Families

Staff work closely with families to make the transition process clear and collaborative. This includes custom Transition To-Do Lists, information sessions and discussion groups. Families also have access to a Transition Portal, which offers guidance on key topics such as housing, employment, application to adult services, SSI/SSDI, and guardianship. Additionally, in partnership with our Family Education and Support program we provide a Transition Speaker Series bringing professionals in the field to discuss topics relevant to planning for next steps after Riverview, and a facilitated small-group Alumni Mentorship program for families who are in their final year of the GROW program.

Annual Transition Weekend

Annual Transition Weekend is a weekend-long event held yearly, filled with information sessions, panels, and opportunities to connect with the Riverview community.

From the moment a student joins Riverview, transition planning becomes a part of the curriculum. It’s not a single step, but a thoughtful, ongoing process that evolves to align with current needs and future goals.

Middle School – Discovering Interests and Strengths

Students and their families are supported through a comprehensive transition program that begins with assessments of strengths, interests, and goals, leading to the development of a personalized Vision Statement. The middle school curriculum includes project-based and transition-focused learning, enriched by field trips, community outings, and activities designed to build independence and executive functioning. The focus remains on giving them the foundational building blocks to set them up for future successes in school and beyond.


High School – Connecting Strengths & Experiences

The high school transition model focuses on providing a well-rounded approach to preparing students for life beyond school, beginning with a personalized Vision Statement, based on assessments of their strengths, interests, and goals. The high school curriculum is designed to meet students where they’re at to find the best educational plan, and students engage in field trips, activities, pre-vocational experiences, and community outings that promote independence and strengthen executive functioning. Students and families work with our team to envision and plan for a happy, successful life after graduation. 


GROW – Planning for Life After Riverview

During our transition program, GROW (Getting Ready for the Outside World), transition services are highly focused on providing students and families with a personalized and practical pathway to adulthood. We start with assessments of strengths, interests, and goals that inform the creation of a tailored Vision Statement, making sure that every student’s pathway is tailored to ensure a happy and productive life after graduation. Students gain hands-on experience and executive functioning skills through meaningful vocational training and internships, while also developing personal portfolios to showcase their growth and achievements. Alongside their vocational experiences, students continue to further their education through independent skill development, core classes, and the opportunity to attend classes at the local community college. Riverview works in direct collaboration with adult service providers, including state, local, and private agencies, to provide a pathway to navigating life after school.

Transition Speaker series

Designed for families who have students aged 16 and up. Professionals in the field present on topics relevant to Transition Planning. Follow-up discussion groups occur the following week, allowing families time to further absorb and discuss the topics together.
